Measure from the person’s perspective and experience:
Measures must provide an opportunity for the person to express a preference, desire, need, or want, and indicate the degree to which these have been met
The person with a disability must be encouraged to answer the questions from their own perspective rather than someone answering on their behalf unless they are not able to respond to the questions.
Measurement information collected from someone other than the person with a disability is not person-centered.
