Referral pathways
Referral pathways: When screening identifies developmental concerns, DON’T WAIT, refer for additional evaluation and supports.
- Education pathway: Call 1-866-693-GROW(4769) or visit HelpMeGrowMN.org to make a referral (no medical diagnosis is necessary and services typically start between 30-45 days)
- EIDBI pathway: EIDBI referral form for a medical diagnosis. A comprehensive multi-disciplinary evaluation medical diagnosis is required for EIDBI services in the home or community based on medical needs.
- *Note: A medical diagnosis may help a family access healthcare coverage & additional services.
- *If the child/ family is not currently on MN Medicaid Medical Assistance (MA), Contact the family’s local county or tribal office to apply for MA
- *IQ and adaptive skills testing are required for certain Home and community-based services, such as case management. Request that the provider include these tests as part of their evaluation or make a referral.
Eligibility for EIDBI Services:
- Birth -21 years
- Medical Necessity Eligibility through EIDBI
- Medical Assistance (MA), MinnesotaCare, MA-TEFRA, Pre-paid health plans, Private insurance, Waiver is NOT required and does not prevent someone from accessing EIDBI
- Use the EIDBI referral form .Referral: Anyone can make a referral.
