OVERVIEW: The Monthly Fidelity Survey is a resource for C&C coordinators to assess the quality of a mentor's data entries.

C&C recommends that coordinators collect and review a monthly sample or "snapshot" of monitoring forms and/or data from the C&C App.

How do I use the Monthly Fidelity Survey to support PD?

Using the data from the Monthly Fidelity Survey can assist the C&C coordinator in:

• Identifying any mentors struggling to implement with fidelity
• Areas for skill development
• Barriers to implementation

Discussing these results at monthly meetings (or individually) will guide the coordinator to design PD targeted to increase a mentor's fidelity of implementation.
