“Check” Data

You’ve been collecting “Check” data about Sophia for a month now, and building your relationship during weekly “Connect” meetings.

With a month of data, it is time to assess for risk of disengagement. Identifying an increased level of risk directs the focus of an intervention.

The next three slides will display the information you have about Sophia’s “Check” data and ask you to determine where there are risk factors and what you would do next.
