These modules are designed to help you create a plan to make changes in routines and in the everyday work within HCBS. Many teams implementing person-centered practices report that it takes a few years to make the changes that are needed. The goal is to add a few goals each year that can be easily completed and that does not create stress.
Teams that successfully change person-centered practices are:
Using an action plan that is reasonable and achievable,
“Working smarter, not harder” by building practices into everyday meetings, work routines, and processes, and
Setting their own pace and thinking about the person-centered change process as “a marathon, not a sprint.”
