  • What are you pleased about?
  • What are you concerned about?

The Person-Centered Organizational Development Tool was developed as an action-planning process to help services become more person-centered. Once a group has completed the survey items, each area includes steps for problem solving. You will see a table with one question in each area:

  • What have you tried?
  • What have you learned?
  • What are you pleased about?
  • What are you concerned about?

People are encouraged to write down responses in each area. In the next section, the responses that are recorded are used to make decisions about moving forward including:

  • Action Steps,
  • Current Partners,
  • Potential Partners, and
  • Technical Assistance Needs.
