There is paperwork that every person who is hired may be required to complete. The financial management services agency has a packet of information and forms for DSWs to complete. If you don't have a financial management services agency or are unsure of this process in your state, check with your service coordinator for assistance. It is important to make sure all of the paperwork your financial management services agency gives you for the new employee is filled out and returned right away. This packet may include, but is not limited to:
- Federal or state background check permission form
- US CIS Form I-9, Employee Eligibility Verification form, which requires
- Identification with a picture
- Employment Authorization document
- IRS Form W-4, Employee’s Withholding Certification
- State Form W-4, Employee’s Withholding Certification
- Time sheet due date and payday schedule
- Direct deposit form (if the DSW wants to be paid via direct deposit)
- Timesheet and instructions
- State family and/or sick leave notice, if applicable in a state
