If your DSW isn't doing their job the way you want, keep track of what they are doing and when you talk to them about their poor performance.  Keep documentation of poor performance. Include the following information:

• Your concern about their performance (such as being late or not following your care plan)
• Any feedback you offered to improve their performance
• Any written performance improvement plans

Make sure to include dates on all records.
