Brainstorm Long-Term Planning
The team will have a lot of ideas for moving forward after completing the Vision Boards and Outcome Statements activity. Some of these ideas are very complex and may take years to achieve. Other ideas are simple and can be achieved within a year. There may be different opinions about what can be achieved in one year and how the goals can be completed. Prioritizing ideas together can be one of the hardest steps the team will take moving forward.
Tips for Prioritizing
- Start by prioritizing the ideas the team identified from Module 4 that can be easily completed now or in the first year,
- Record the ideas that the team can complete over the course of Year 1 in Appendix C (under Year 1),
- Identify the ideas that may take more time to complete; add those ideas to Year 2, Year 3, or your long-term plan in Appendix C (review the section in the HCBS Planning Tool the team completed in Module 4 under the “need more information” section), and
- Add any new ideas that come up and place them in Year 1, Year 2, Year 3 long-term plan based on the amount of time the team believes will be necessary to complete the work involved.
Discuss as a team what will happen once each idea listed in Year 1 is completed. Write down ideas for the next steps that would naturally occur in Years 2 and 3 in order to continue forward momentum toward the team’s Outcome Statements.
Activity
Checklist item 16: The team uses assessments to agree on a reasonable list of goals and activities for the year.
