Finalize the Long-Term Plan

Item 18 on the Minnesota Team Checklist asks teams to develop a long-term plan for expanding person-centered and positive support practices. The long-term goal provides a formal way write down what your team decided for Year 1, Year 2, and Year 3, and includes the SMARTIE goals your team created for Year 1. These details are considered your long-term plan.
Share the initial draft of the plan and gather feedback from staff and others. The team can use this added feedback to create a final draft of ideas for the long-term plan. To save time, it can be helpful if one or two people work on the final draft instead of writing together as a larger group.
Review the plan during the year to help the team to keep thinking about the Outcome Statements and vision. It is okay if the team decides to change the long-term plan ideas over time. Progress on the Year 1 action plan will help the team think about and make changes to the long-term plan for moving forward.

Review your Vision Boards, Outcome Statements, and Action Plan for Year 1, Year 2, Year 3, and Long-Term priorities. Do they all align and support your vision to expand person-centered and positive support practices?
