Disseminate Final Action Plan to All Stakeholders
Item 19 on the Minnesota Team Checklist asks the team to share the final version of the action plan and long-term plan with all stakeholders. The way teams share details about the plan with stakeholders may vary for smaller and larger HCBS teams. Information can be shared with other stakeholders in the same ways that were described earlier including the examples below.
- Schedule a dedicated meeting to talk about the plan
- Add agenda items to events that are already scheduled with different groups
- Send an email with a request for feedback
- Share the documents including an online survey to gather ideas and guidance
- Post the information on a website so that people can learn more about the details of the plan and send feedback to the team
Some teams combine the work involved in Items 17 (include all staff) and Item 19 (include all stakeholders) by bringing people together in one step setting up an onsite or virtual event or by sharing information with people using a combination of email, website, and online surveys.
The team may choose to share all of the final draft documents or create a shorter summary of the results based on the following:
- Photos of the Now and Future Vision Boards,
- Outcome Statements,
- Action Plan, and
- Long-Term Plan.

Checklist item 19: Plan is in place for sharing the action plan with all stakeholders (e.g. , people using services, their families, guardians, staff, case managers, community members).
Review the plan you developed in Module 4, Item 15, and the plan you just created, and make arrangements to share with staff and all stakeholders.
