Creating the Evaluation Plan
Now that we have reviewed effort, fidelity of implementation, and outcome data, your team can begin to organize an evaluation plan. The outcome statements and long-term plan the team worked on in Module 5 help guide the team’s evaluation. Outcome statements and the long-term plan help establish why the team chooses certain measures to assess progress. Include the outcome statements, the long-term plan, and the Year 1 action plan, Appendix C, in your evaluation plan. These documents are part of the team’s overall evaluation plan.
The first evaluation question prompts the team to document person-centered practices, such as person-centered thinking, Charting the LifeCourse, PATH or other person-centered strategies. The team also includes:
- Positive supports being used in HCBS,
- Types of information that help document effort,
- People who organize this information,
- Frequency of data collection, and
- Meetings in which the data are used to make decisions.
Visit the Module 7 Resource Page for an example of an evaluation plan.
