Creating the Evaluation Plan

Now that we have reviewed effort, fidelity of implementation, and outcome data, your team can begin to organize an evaluation plan. The outcome statements and long-term plan the team worked on in Module 5 help guide the team’s evaluation. Outcome statements and the long-term plan help establish why the team chooses certain measures to assess progress. Include the outcome statements, the long-term plan, and the Year 1 action plan, Appendix C, in your evaluation plan. These documents are part of the team’s overall evaluation plan.

The first evaluation question prompts the team to document person-centered practices, such as person-centered thinking, Charting the LifeCourse, PATH or other person-centered strategies. The team also includes:

  • Positive supports being used in HCBS,
  • Types of information that help document effort,
  • People who organize this information,
  • Frequency of data collection, and
  • Meetings in which the data are used to make decisions.

Please use the MN Team Checklist and the HCBS Planning Tool to complete the activity.

Minnesota Team Checklist.  Item number 33 in the Evaluation section.  An evaluation plan is in place for person-centered and positive support practices:

Examples of data include:

  • Minnesota team checklist and other fidelity of implementation data
  • Incident reports, 911 calls or other emergency events, injuries, etc.
  • Staff attrition, retention, satisfaction, sick days, etc.
  • Quality of life, satisfaction levels, development of new skills, etc.

Score the checklist: 0 – planning not yet started, 1 – in progress, or 2 – fully in place.
