Fidelity of Implementation Data
These modules are used to help your organization proceed with any type of person-centered or positive practice. A team may choose to implement one or more practices. Keep track of how well each of these individual practices are being implemented. For instance, one story in this module described an organization implementing both person-centered thinking and positive behavior support. The TOET was used to evaluate how the team was implementing both practices by using subscales that focus on the key elements.
Item 35 on the Minnesota Team Checklist guides teams through the overall process for implementing person-centered and positive support practices. Each practice includes a list of key elements for implementing it as intended. More information can be found in the Module 7 Resource Page .

Please use the MN Team Checklist and the HCBS Planning Tool to complete the activity.
Minnesota Team Checklist. Item number 35 in the Evaluation section. Fidelity of implementation data are gathered overall using this tool and for each type of person-centered and positive support practice.
Score the checklist: 0 – planning not yet started, 1 – in progress, or 2 – fully in place.
