Evaluating Fidelity of Implementation at Tier 1
The second type of evaluation in HCBS is called Fidelity of Implementation. Fidelity of implementation is a way to assess whether person-centered practices or other positive supports are being used effectively. You have been introduced to one tool already that guides teams through Tier 1 evaluation, the Minnesota Team Checklist. This checklist is referred to as a self-assessment because teams decide what the score is for each item.
There are 36 items on the checklist. Teams that score an item on the checklist as not started receive zero points for that item. If an item is scored as “in progress,” it is worth one point. Each completed item is worth 2 points . This graph shows progress by subscales, which are the groups of items that related to a checklist topic. The bar graphs show the different times that checklists were completed. A total score is available on the right side of the graph.
