Create a Plan to Share and Receive Feedback from Everyone
The only way to become a more person-centered organization is for everyone to work together. Item 3 on the Minnesota Team Checklist prompts the team to create a plan to involve more people over time. The plan usually includes a list of ideas for meeting and sharing information that will work best for the services provided. Teams assess the strategies that are already in place within HCBS that can be used. New ways to provide details about person-centered practices and to gather advice from others are also added to keep people informed.
Providers choose a number of ways to share information and gather feedback. Information can sent out to people in a way that does not require bringing everyone together to meet all at once. Emails, newsletters, website links, and surveys can be used to decrease the costs of meetings. Information can be shared with workgroups and events that are already scheduled for other reasons to save resources.
Ask your team members and others how to involve different types of people in this process in a way that works best for your HCBS.
- Add time to staff meeting agendas
- Visit home/family during meeting times
- Create an event to share with a larger group each year
- Share updates in emails, newsletter, virtual meetings or social media
- Create a photo journal that can be used to talk to people using plain language
- Talk to each team member during phone calls and/or meetings
- Visit homes where services are provided and talk to the people receiving services and staff members
