Brainstorming Who Should Attend Person-Centered Practice Meetings

The first item on the Minnesota Team Checklist prompts you to form a team of people who can make changes together. Think about people that might be invited to join a team. If a team for person-centered practices already exists in your organization, are there any people missing who can offer new insights based on their role? Are there other people who can be added to the team to represent missing voices?

The Minnesota Team Checklist can be used each time your team meets, or you might choose to complete this tool on a quarterly or annual basis. This tool is used as a way to engage in evidence-based practices. If your team’s score on the Minnesota Team Checklist improves over time, you are showing evidence that the services provided are becoming more person-centered.

Examples of Team Members in Different HCBS Settings

Family members, people who receive services (depending upon the size of a provider), director of the facility, coordinator, manager, personal care attendants, volunteers, nurses.

One or more children or adults with autism receiving services (depending upon the size of a provider), family members, teachers, therapists, director of the health care-provider organization who is guiding in-home HCBS services.

One or more persons with IDD (depending upon the size of a provider), family members, staff members who provide direct services, director, manager and human resource staff, nurses, case manager, community members.

Please use the MN Team Checklist and the HCBS Planning Tool to complete the activity. 

Minnesota Team Checklist.  Item number 1 in Team section.  A team is in place to work toward becoming a more person-centered service. Score the checklist: 0 – planning not yet started, 1 – in progress, or 2 – fully in place.

Use the Team Brainstorming Worksheet, Appendix  A,  to make a list of the people you might invite to be part of a person-centered team. Create a wish list that you will use to reach out to invite possible team members.
